Senior CareLinks

Senior CareLinks is designed to improve the quality of life for seniors in our community by helping them manage their health care needs.

Directed by a Family Nurse Practitioner, Senior CareLinks is a one-stop connection to a full range of services needed to help seniors live a healthy life.  Family Nurse Practitioners work with patients, families, physicians, long-term care providers and Tulare District Hospital to arrange or provide the services needed. Services include:

• Physician Referrals
• House Calls (physician directed)
• Referrals for home health, long-term care or home medical equipment
• Advance Planning for important life choices
• Insurance Counseling
• Diabetes Management
• Cardio/Respiratory Health
• Nutritional Services
• Prescription Advice
• Health and Wellness Classes
• Support Groups
• Medication Management Program
